Sliding fee scale policy
Netra Integrative Health Clinic will see all patients regardless of the ability to pay. We offer a sliding fee discount based on family size and income.
Sliding fee discounts are available to patients with incomes at or below 200% of the Federal Poverty Guidelines, as published and updated annually in the Federal Register, with reduced assistance offered up to 250%. Discounts are applied to our standard fees.
Senior discount
Senior citizens receive a 50% discount.
How we calculate household income
For the Sliding Fee Discount Program, annual household income is based on the household’s combined federal Adjusted Gross Income (AGI).
If you provide a federal income tax return, use the amount reported on Form 1040, Line 11. If more than one household member files a tax return, the Adjusted Gross Income from each applicable return may be combined. Do not use taxable income from Form 1040, Line 15.
For self-employed applicants, we use the net business income included in Adjusted Gross Income after allowable business expenses. Gross business sales or receipts are not treated as household income.
Because eligibility is based on current-year income, we may request recent pay stubs, a year-to-date profit-and-loss statement, benefit statements or a signed income attestation when current income differs materially from the most recent tax return.

Ask our office for the Sliding Fee Discount Application form and bring it to your next visit along with any of the following items:
- W-2 form
- Recent tax returns
- Last two pay stubs
- Social Security statement
- Unemployment check stubs

The chart reflects the HHS Federal Poverty Guidelines for 2026 for the 48 contiguous states and Washington, D.C. For household sizes above 4, add the 2026 per-person increment of $5,680 to the 100% FPL figure and scale by the band percentage.

Discounts are applied to standard fees. Eligibility is determined by household income and family size as defined by the current year’s Federal Poverty Guidelines. Income verification, such as pay stubs, tax returns or a signed attestation, may be required.
Health insurance
Medical insurance is not required to receive services at Netra Integrative Health Clinic. Acupuncture is also a qualified expense for HSA, FSA and HRA plans, so you can put those accounts toward your sessions with us.
